New Student Health 101

Student Health 101 is an online monthly magazine aimed at student health and wellness promotion and more. It is available to the students of St. Lawrence College through the collaborative efforts of Student Services, and the Brockville and Kingston Student governments. Confidentiality

  • All services provided in Campus Health Services are confidential. Health Records are entirely separate from other College documents. NO access to your health record by an academic or administrative department. Access to any information must be obtained with your written consent.
  • For those students enrolled in programs with immunization requirements a CONSENT FOR RELEASE OF IMMUNIZATION must be signed in order for your records to be released to placement agencies, student service representatives, program co-coordinators, faculty, Deans, Associate Deans and the Public Health Department as required.

IMMUNIZATION - COMMUNICABLE DISEASE Requirements

Completion of the IMMUNIZATION - COMMUNICABLE DISEASE FORM is a MANDATORY condition
for laboratory/practical placements for many programs at St. Lawrence College.

Step 1: Download your IMMUNIZATION - COMMUNICABLE DISEASE FORM (pdf), from your program.
Step 2: Complete the requirements and submit the form. This form is your responsibility.

Note:  Full-time students coverage for vaccines received on or after September 1, can be claimed under the Student Sickness & Accident Insurance Plan.
